Sun-Times Chairman James Tyree suffered from cancer and pneumonia; however, his death was the result of a routine medical procedure, according to the Cook County medical examiner’s office. The cause of Tyree’s death left his family and friends stunned.

Tyree was diagnosed with stomach cancer and had recently been admitted to the University of Chicago Medical Center with pneumonia before his death. Two hours after being admitted, the 53-year-old began experiencing problems following the removal of a catheter due to an air embolism. Tyree was rushed to the hospital’s intensive care unit, where he remained until his death. Tyree’s cause of death was listed as death from air embolism.

The Dangers of Air Embolisms and Why They Should Never Happen

An air embolism is the obstruction of a vein or artery by an air bubble, and often occurs during medical procedures. The bubble is often harmlessly absorbed into the blood, but a more serious blockage can shut off the flow of blood to the brain or heart, causing injury or death. An embolism is not always fatal, but a person battling serious health problems involving the heart or lungs is at higher risk, said Dr. Robert M. Wachter, associate chairman of the department of medicine at the University of California at San Francisco Medical Center.

Death from an “intravascular air embolism” at a health care facility falls into a category of medical errors referred to as “never events” because they are considered preventable situations that should never occur, according to The National Quality Forum, a medical standards organization.
